Click here to download the application form. If you don't have Adobe Acrobat, click here to get a free copy of Acrobat Reader. (Notes: Mac users can use Preview. To fill out a PDF form in Adobe Acrobat, select the hand tool as opposed to the cursor tool.)

Tab between form fields to enter your responses, and be sure to use "Save As..." to save the filled-out form as a new document with a different name. Participants are usually required to e-mail the form and their scripts a month before the program begins.
